Our daughter is getting a Spica Cast tomorrow for hip dysplasia and her surgeon had no answers about car seats either. There is a seat I found through a TON of research that's available on Amazon Prime. It's slim and super sturdy with low wings on the sides. Our surgeon actually asked me for the brand because another family had a similar story to yours last week too.
Reading your post makes me feel not so alone as we prepare for 10 weeks in that stupid cast. Hang in there! :)

The Diono Radian and Diono Rainier are pretty much the same thing and both good options. Unfortunately our girl only fit in a Hippo (?Britax brand) and even then it was *just* able to fit with the second cast. I may be slightly nervous that she won't fit our Radian after her next surgery (and no Hippo to loan from the hospital ).
I’m so sorry to hear this! The spica casts are awful but kids are so much stronger than we give them credit for! Hope everything went smoothly and your little girl is doing good! I just posted my tips and items we used to make the weeks seem less daunting. Hope this helps!
